A standalone expansion of Painkiller

Painkiller: Overdose is the second stand-alone Painkiller game. The story is set just after Daniel kills Lucifer. The death of Hell's leader allows Belial, the prisoner and outcast, to escape and start his vendetta against those who imprisoned him: demonic beast Cerberus and Sammael, an angel who betrayed his own kind. Belial starts his journey with killing his prison warden (and later using his head as a weapon). After traveling through various hellish places and defeating countless demons he kills Cerberus and reaches Samael's lair. There, Belial defeats the fallen angel and rips his wings off as an act of vengeance.

Great (standalone) expansion for the Painkiller series.

Painkiller: Overdose is a standalone game that takes place in the events before Painkiller: Battle Out of Hell. Daniel just killed Lucifer and the effects of this action spawned the events of this game.

In this game, you play as the half-angel and half-demon Belial, a gatekeeper that has been imprisoned for all eternity. When Lucifer gets killed, Belial was automatically released from his prison and now you only have one goal: revenge on those that imprisoned you. This includes the Warden, Cerberus and the Angel Samuel (from the first Painkiller game) that betrayed his own kind and is directly responsible for your imprisonment.
You first kill the warden and use his head as a weapon. It can shoot special projectiles that are really strong and epic to use. Then, you move on to the hound Cerberus and finally, Samuel. When you defeat him, you rip off his wings as an ultimate statement of vengeance.

The game plays the same as the first games but feels unique because of the new Hellish level design, the many, many new enemies and new weapons like the crossbow, which is overpowered as Hell (pun intended).

In terms of graphics, sound and controls, nothing changed much. It looks a little better and more polished, the sound is just as great as the first two installments and the controls are just as fluent. The only shame is that the levels do not feel that unique anymore. It all looks kind of the same, with the same orange tint in the air. Also, the music disappointed in this game. Not every game can has the energizing metal tracks from the first Painkiller game, but you are still playing Painkiller and shoot the living crap out of demons, so it was a shame that the music downgraded this much.

There are however some differences and issues with this game. For example, the ammo is extremely scarce. In the previous Painkiller games, you got a fair amount of ammunition that you can find on the levels, with Overdose, you cannot waste any shot, or you run out of ammo. Many times, I asked myself: “There has to be a crate somewhere, right?” The answer was no in most cases.

Also, a special note must be made for the loading times. The leap from the first game to this one is surprising. You literally have enough time to take a sh!t or make a cup of coffee by the time this game has loaded, even on fast hardware and SSD’s.

The game also feels a little buggy and unstable in comparison, but this could be explained by the different teams (while supported by the original developers) who worked on the game.

Besides its flaws, I still think Painkiller: Overdose is a good game that offers some variation in comparison with the main game. The combat and weapons are fun, the enemy types weirder than ever and the action is just as intense (if you have ammo that is).

Still recommend it.

Painkiller Overdose has to be one of my favorite Painkiller Games of all time besides its the first expansion pack of Painkiller, the weapons, graphics, music, its protagonist, Belial, gameplay, and everything in is so awesome, which cooked very well from the first game (black edition), Painkiller Overdose is a perfect painkiller expansion pack better than resurrection.

Painkiller Overdose is the first expansion pack for Painkiller made by a studio other than People Can Fly and jesus christ does it show.

Overdose has some good in it, such as the beautiful looking levels and awesome new enemy designs. The new weapons steal the show as while most are reskins of Daniel's arsenal in the original game, they are still fun to use.

But that's where the good ends because this game is an unbalanced mess. For starters, the game gives you little ammo at all. Many times I found myself out of shotgun ammo which I never had the problem with in the original game. Some levels you would play through its entirety with no ammo for 3 of your guns because there are no pickups for them.

Another issue is that there are so many obnoxious enemies in the game which they throw at you in full force. In the original Painkiller, I would struggle to think of 5 enemies that I found extreme annoyances whereas in Overdose, there's annoying enemies spawned in everywhere and it makes me wonder if the developers actually playtested some of the levels. You've got enemies which teleport or spawn right on top of you, enemies that home into you and you cannot dodge them, or enemies that have a gas spray attack that has a huge hitbox that even when you're not in the actual particle effect, you still get hit anyway.

Another problem are the tarot card requirements. In the original Painkiller, tarot card requirements made sense for each level allocated and felt like a good challenge rather than a complete slog. Battle Out of Hell had the problem with making the tarot card requirements the hardest possible feat to pull off in each level in order to unlock the card. Overdose on the other hand just throws in tarot card requirements for levels and it's obvious that they didn't bother testing them to see if they "fit". Short levels in Overdose for some reason have tarot card requirements such as "Beat the level in 20 minutes" and without even rushing through the level you can beat the level before you hit the 10 minute mark. The worst offender is the Dead Marsh level, which wants you to only use your melee weapon for the entire level if you want to unlock the card. The problem is that the enemies in the level are extremely dangerous at melee range and have enough health to tank a few hits from the ranged attack of the melee weapon. At one point, the level spawns in flying enemies with tiny hurtboxes and fly erratically around the player. It becomes incredibly tedious to try to kill the 30 or so flying enemies with your melee weapon that I doubt anyone who made the level actually tested the tarot card requirement to see if it's reasonable. The asylum level in the original level had the same melee only card requirement but it fit the level because it was indoors, enemies moved very slowly and they didn't have a lot of health so you could use the ranged alt-fire to kill them with ease.

My final issue with the game is the performance. This game has so many crashing issues. Some tarot cards are impossible to place in your deck because it causes the game to crash instantly and entering demon morph mode is like a gamble whether the game wants to crash or not. The worst are the fucking horrendous loading times. Even with the fix I used from a guide, the loading times are awful. You could probably make a sandwich or go for a toilet break and when you get back the game still hasn't loaded a level yet. This makes death even more frustrating because you have to sit through another long loading screen again.

Overall, Painkiller Overdose is mid at best and only diehard Painkiller fans would only find some enjoyment out of this with a protagonist with the ultimate ability to repeat the same 4 one-liners 37 times a level.

In defense of the game’s formula, you really have to be an FPS nut, or a fan of the classic games like Doom and Quake, to appreciate what’s going on here. There are no cinematic events, no quick time events, or helicopter rides. This is just pure shooting with enemies doing nothing but coming after you. If you played the last game and its expansion and got tired of it you probably want to come back again because of the interesting and completely original levels and enemies. That’s probably the only saving grace for Overdose, but it does have some niceties.


First off the story is semi-interesting with you playing as a half demon/half angel who gets thrown into a prison in Hell for thousands of years by Samuel and Cerberus. You ascend through Hell, up to the middle plane, then up into Heaven. The levels are just awesome and so original like Dead Marsh which is inspired by H.P Lovecraft with Cthulhu type enemies. Riot is a level with creepy looking police, hoodlums, and sexy nurses. Farmland has creepy reanimated butchered animals, and the list goes on but the gory mature nature of Painkiller stays. I liked this game over the last more because of these awesome new levels and enemies that are drastically different from before and never feel repetitive.


The weapons are all new with an Evil Eye which is a severed demon head that shoots a laser eye. You get a cannonball launcher with a machine gun attached, crossbow, radiation launcher, and just a bunch of cool weapons that make the game feel completely new. Of course, you get to collect coins in the game and use it on tarot cards that act as perks which you will really need if you aren’t playing on Daydream difficulty.


That is also the main problem with Overdose is that it’s ten times harder than the last game. You will die hundreds of times throughout the game, and I couldn’t beat it on anything higher than Daydream because it’s just too damn hard. My only other comment is that the character you play as chimes in every so often with funny comments and will react to the level you are in when you enter it. Despite all this, the game is just running around and shooting, end of story. It can get boring really quick and only hardcore FPS fans should stray into the Painkiller realm because it’s damn hard. Unfortunately, online play is pretty much dead now, but we do get updated visuals and it looks good even for its age. Stay away if you don’t like tough shooters, come back if you love Painkiller.
