Games Featuring Transgender Characters
Inclusive of nonbinary/gender neutral characters. Please be aware that not all of these games will feature positive depictions of trans people and that some notes may contain spoilers.
If you know of a game not listed, please mention it to me!
** I choose not to include characters who are considered intersex, since not all intersex people consider themselves transgender. Exceptions are only made if the character is stated or considered to be intersex as well as transgender.
** I also choose not to include the titles Hogwarts Legacy and The Last of Us: Part II. While these games do have trans characters in them, due to the fact that JKR (who I am aware was not directly involved, though I still feel that giving the game any positive attention would still benefit her) and Neil Druckmann are enormous bigots who have their bigoted views written directly into the stories of their creations, and that they have both used money they've made off their IPs to fund harmful charities, I do not believe either of these people should be supported, monetarily or otherwise. I am not changing my mind about this and anyone who attempts to argue with me will be ignored, you are just wasting time for us both. Create your own list if this bothers you.

It is heavily implied Arashi is transfeminine.
According to the Ensemble Stars wiki, while early stories from 2015-2017 had a very different approach to Arashi's character, stories released in 2018 and onward indicate a more nuanced approach toward the subject of gender identity, and have Arashi explicitly talk about "struggling with gender" in the Japanese script.
In the Japanese version of the game, Arashi uses the first-person pronoun Atashi, which is often used by women, and she loves to be referred as Onee-chan ("big sister"), and often uses that title when speaking to people.
Several characters such as Mitsuru, Madara, and Tetora refer to Arashi as "onee-chan," "daughter,” and a "charming woman."
Arashi also says this about her gender and self-love: "No matter how much I want it, and no matter how hard I try... I could never become the beautiful woman I dream to be. But even if that’s true... I won’t let it depress me. I won’t whine about it anymore. I love myself most, no matter who I am."

Liezel's official description talks about her being a trans woman:
"Liezel grew up in the rural town of Scrapple Hill. She was a natural at football, making varsity in her freshman year. Despite the wins and cheers of that season, Liezel never signed back up. The anxiety of trying to fit in with the other boys wore down on her. With each year, she became more quiet and withdrawn. After graduation, she moved to Tastyville where she made many like-minded friends. In time, Liezel became the happy, exuberant woman she always felt herself to be."

From Wikipedia:
In real life, Sweet-P was born male. In Mobius, the alternate World everyone was trapped in, Sweet-P is given the body she desires as a teenage feminine skinny girl. As she now has the body she desires, she does not want to leave Mobius as returning home would have her return to her overweight male body and so goes against the Protagonists, who do want to leave. Sweet-P doesn't want to transition and go through surgery in real life, as she doesn't want to make things hard for her family and also finds it's too late anyway as she is overweight and in her 30s, and as such wouldn't be able to be a skinny teenage girl in real life just like in Mobius. She does still use female pronouns regardless, though almost the entire cast bar a few characters bully her, call her male, and make fun of her for being a "fat overweight man in real life." The protagonist can choose to be supportive, but also has the option to bully her for it as well.

In Alice's final hang out time, Professor Mishima mentions to Alice (who introduced himself as Gonbee in the main game) how the tone of his voice sounds girly sometimes and asks him if he has "the heart of a woman". Alice explains that his mother had raised him as a girl because of the phrase "first a daughter, then a son" meaning that if you have a girl first, you'll have an easier time raising a boy. Alice's mother interpreted this as you should have a girl first because it's trendy, and so raised him as a girl, but while Alice grew up he felt something was wrong and he definitely felt more masculine and told his mother he wanted to live as a boy.

Another romance option, Oli, at first appears to be whichever gender you select, but this turns out to be a false identity for a character named Dhanthik, who is nonbinary and uses zie/zir pronouns.

I have never played this game, but this seems to be a hot topic of debate. Ultimately I think it is best to say that you are welcome to disagree and create your own list if this inclusion bothers you.

Heaven Will Be Mine also has Mercury! (trans man, revealed in backlogs referencing HRT and misgendering) https://hwbm.fandom.com/wiki/Mercury#Background
@sazandorable Thank you, I've updated the note to reflect this!
In Slay the Spire, the Defect (one of the four playable characters) is referred to with it/its. I believe it is canonically genderless but not 100% on an official source on that.
