The World's Hardest Game and The Impossible Quiz are good candidates. There are still some entries from SMW Central if you want to add: Super Dram World 1 and 2, Banzai Mario World, Grand Poo World 1, 2 and upcoming 3, Invictus. All of them are SMW kaizo romhacks. Super Mario 64: Chaos Edition also can make it here to some extent, but maybe not, since difficulty comes from random effects rather than deliberate level design. For The same reason Spelunky 1 and 2 can't make it here: they are hard and very punishing, but most of the difficulty (and replayability) comes from random generation.
